CNA Program Admission Checklist

1. Completed Student Application Form

2. Valid Government-Issued Photo ID (Submit either one of this)

• Driver’s License

• State ID

• Passport

1. Social Security Card or ITIN Documentation

2. Educational Requirements - Fortress Medical Institute welcomes applicants from diverse educational backgrounds. A hiigh School Diploma or GED is recommended but is not required for enrollment into the Nurse Aide training program. Students must demonstrate abilitly to read, write, speak, and understand English sufficiently to successfully complete the program. 

3. Ability to read, write, speak and understand English.

4. Background Check Clearance 

5. CPR Certification (BLS Preferred)

6. Immunization Records

• TB Test/Chest X-Ray

• Hepatitis B

• MMR (Measles, Mumps, Rubella)

• Varicella (Chickenpox)

• Tdap

• Flu Vaccine (if required by clinical site)

• COVID-19 Vaccine/Exemption (if required by clinical site)

• Drug Screening (if required for clinical placement)

• Physical Examination/Health Clearance Form

1. Tuition Deposit or Payment Arrangement

2. Clinical Requirements Acknowledgment Form

3. Attendance & School Policy Agreement

4. Signed Student Enrollment Agreement
